Individual support provided by individuals with lived experience with mental health or substance use to help with service coordination and advocacy activities as well as rehabilitative services. This is intended to assist with maintaining community tenure and help with daily functioning.
Eligibility Criteria
Full Medicaid or below 200% FPL. May qualify for fee reduction otherwise.
Instructions for Referral
Complete an open access appointment at any Plains Area office by walking in with no appointment M-Th 8am-2:30pm; F 8am-1:30pm.
