Services are designed to support individuals as they live and work in the community. Services address mental and functional disabilities that negatively affect integration and stability in the community. Staff attempt to reduce or manage symptoms that result from a mental illness.
Eligibility Criteria
Individuals with severe and persistent mental illness. Full Medicaid or below 200% FPL. May qualify for fee reduction otherwise. Not on habilitation waiver.
Instructions for Referral
Complete an open access appointment at any Plains Area office by walking in with no appointment, M-Th 8am-2:30pm; F 8am-1:30pm.
