At Hope Haven in Spencer, we provide disability and mental health services, including My Day (day habilitation), community-based skill building, and behavioral health supports helping individuals strengthen daily living skills, build social connections, manage mental health needs, and grow in confidence and independence within their community.

Eligibility Criteria
To receive services from Hope Haven, an individual must have a documented mental, physical, intellectual, or psychiatric disability that impacts daily functioning and be able to benefit from and participate in services. The individual (or legal guardian) must consent to services, have an approved funding source such as Medicaid or a waiver, and meet program-specific requirements. Individuals must not pose a serious threat to themselves or others, must be free from active substance abuse at admission, and may be subject to restrictions related to recent serious criminal convictions depending on the program.
Instructions for Referral

To refer someone to Hope Haven, contact the Admissions Office by calling 712-264-2035 or toll-free at 844-303-1070, emailing kkleespies@hopehaven.org, or faxing 712-262-4202. A provider, caregiver, guardian, or the individual may also complete the online referral form on Hope Haven’s website. The admissions team will then review eligibility, discuss needed documentation and assessments, verify funding sources such as Medicaid or waivers, and guide the next steps for enrollment in the appropriate program.
